UVSA-Midwest is committed to taking all reasonable steps to prevent harassment and abuse and will make every reasonable effort promptly and completely to address and correct any misconduct that may occur. Any individual is able to submit an incident report, an internal review request, and/or a negligence report knowing our Executive Board will do our best to resolve the incident and maintain a safe community.
All forms may be submitted anonymously and information will remain confidential. For more information, please refer to our Safe Community Initiative
Use this form to report an incident that has involved UVSA-Midwest and/or its members.
Use this form to report any new information pertaining to an existing incident case.
Use this form to report any negligent actions of any Internal Staff member or Investigation Committee member.
